From the east, LA 158 begins as a state highway just northeast of Colfax. It heads westward from Iatt Lake and intersects US 71. It turns southward, showing distance signs for Colfax and Boyce. The road turns due south and enters Colfax, after passing Red River Lock and Dam. Shortly after an intersection with Faircloth St. (Louisiana Highway 490 Spur), the route terminates at a junction with LA 8.
LA 158 is an undivided, two-lane highway for its entire length. The route was the prior route of U.S. Highway 71 through Colfax, and north to the current US 71 junction. Before the 1955 renumbering, LA 158 was State Route 2116.
The entire route is in Grant Parish.
